Precision Diagnosis and Dynamic Risk Stratification in Portal Hypertension

Description

The management of portal hypertension is rapidly evolving from static staging paradigms toward dynamic, individualized risk assessment, evolving guidelines and deeper understanding of treatment response and disease regression. Novel non-invasive tools, machine-learning algorithms, and longitudinal measurements now allow clinicians to diagnose clinically significant portal hypertension (CSPH), predict decompensation, and monitor treatment response without reliance on invasive hepatic venous pressure gradient measurements. This session will highlight contemporary data validating Baveno VIII criteria, emerging artificial intelligence approaches, perioperative management in advanced liver disease, and the growing role of serial non-invasive testing in identifying patients most likely to benefit from targeted therapies.

Objectives

  • Evaluate the performance and limitations of Baveno VIII criteria for diagnosing CSPH across diverse patient populations
  • Interpret real-world evidence on endoscopic surveillance and non-selective beta-blocker use across successive Baveno eras and evaluate their impact on variceal hemorrhage outcomes
  • Compare emerging non-invasive and machine learningbased approaches for detecting portal hypertension and predicting clinical outcomes
  • Discuss the prognostic significance of regression of portal hypertension and methods to assess resolution over time, including the role of serial liver and spleen stiffness measurements in monitoring therapeutic response
  • Apply updated approaches to perioperative risk assessment in patients with cirrhosis
