Description

Expert facutly review important considerations and updates regarding the management of primary biliary cholangitis (PBC), primary sclerosing cholangitis (PSC), intrahepatic cholestasis of pregnancy (ICP), and autoimmune hepatitis (AIH) in pregnancy through case-based discussion. The PBC case highlights the approach to preconception counseling of an individual on second-line PBC therapy. The ICP case highlights the association of chronic liver disease with ICP, the relevance of genetic testing, and approaches to management from ursodiol to novel secondary treatments including rifampicin and ileal bile acid transporter (IBAT) inhibitors. The AIH case focuses on key considerations across preconception, pregnancy, and postpartum periods including risk stratification, medication safety, and strategies to optimize maternal and fetal outcomes.

Objectives

  • Describe approaches to risk stratify for intrahepatic cholestasis of pregnancy (ICP) in subsequent pregnancies including genetic testing for ICP.
  • List the key components of preconception counseling for women with autoimmune hepatitis, primary biliary cholangitis, and primary sclerosing cholangitis.
  • Discuss approaches to management of complex cases of autoimmune hepatitis, primary biliary cholangitis, primary sclerosing cholangitis, and ICP during pregnancy.
