Objectives

  • Link early critical-care decisions to downstream outcomesincluding survival, functional recovery, and post intensive care unit (ICU) quality of lifeand determine when acute-on-chronic liver failure (ACLF) consensus criteria should drive care versus when individualized clinical judgment, patient goals, and local resource considerations should take precedence.
  • Identify key changes in the 2026 Global ACLF Consensus. Explain how revised organ failure definitions modify ACLF classification and prognosis, and apply these definitions within a trajectorybased framework to guide ICU escalation, reassessment, and transplant decisionmaking.
  • Incorporate understanding of immune dysfunction into ACLF management decisions, recognizing its impact on infection vulnerability, organ failure progression, and responsiveness to therapies.
